Did you succeed in achieving your action steps since the last meeting? If yes, please state which action steps you were able to complete. If no, please explain the challenges that you experienced. (If first meeting, write ‘first meeting’ here.):
Yes, yet, the overall goal is a work in progress. • Still drinking close to two bottles of water per day, would like to increase to three bottles starting April 22nd. • Steps for the previous week have ranged between 6,500-10,200 steps. Has been walking a minimum of 20 minutes per day. Has increased to two cardio activities per week. • Next week, will incorporate more strength training and cardio opportunities. • Must find time to eat healthy snack options. Last weekend, meal prepped for the week. • Still awaiting the dietician appointment. • Will have doctor’s appointment in August to determine if efforts are successful
Please state your goal using the SMART Method: Specific: Avoid generalities! Your goal should have specific details so you know what you are achieving. Measurable: Make sure you have solid criteria for establishing your progress toward your goal. Actionable: You can clearly define action steps towards your goal Rewarding: Your goal must be something that is meaningful and worthwhile. Time Specific: Having specific dates for beginning and finishing goals keeps you on track. Example: “My goal is to receive a Sports Internship by May 1st”:
Specific Goal: Lower cholesterol and weight Measurement: Lower cholesterol by 50% and weight by 31% Actionable: • Exercise 30-60 minutes four days per week • Slowly increase to 8,000-10,000 steps • Increase strength training and cardio • Get healthy snack options from dietician and Mr. Gold. • Drink a minimum of 32 ounces of water per day. Reward: Living a productive and healthy life and decrease likelihood for illness Time specific: Complete goal by April 15, 2024
Please list the action steps you will take to achieve your goals stated above. These are action steps that will be completed by next meeting. Example: “1. Finish my resume. 2. Talk with my advisor. 3. Submit a sports internship application at two institutions.”:
1. Continue to increase increments of water until I am drinking between 64 ounces or half my body weight. 2. Increase strength training to three times per week. 3. Increase cardio to three times per week. 4. Maintain a step goal of 8,000 to 15,000 steps daily. 5. Continue to meal prep on Sundays to be more successful and health conscious about my eating choices.
How have the NSLS speakers or SNT meetings helped you achieve your goals or changed your life?:
NSLS has helped to jump start my S.M.A.R.T goal. The speakers gave me inspiration to get started. The SNT meetings gave me accountability and motivation to maintain my efforts in obtaining my overall goal. This will change my life if I continue to hold on to what I have learned to live a healthier life.
